Is IFMG closed?

Yes, IFMG closed it’s primary care practice on March 31st, 2026. Family Heath Matters now operates the clinical space.

Can I continue to get care from Dr. Lew, Perlee, or Monika?
Yes, Dr. Lew, Perlee, and Monika continue to see patients out of the Fullerton office as employees of Family Health Matters.

How do I make an appointment with Family Health Matters?

Call Family Health Matters at (714) 441-0411 and request an appointment with Dr. Lew, Perlee, or Monika.

How do I access my medical records?

I, Dr. Clarke Lew, will remain custodian of your medical records for a period of 6 months (April 1-Sept 30) following the closure of the practice. 

If you need a copy of your medical records or to have records transferred to another physician, please contact our office using the phone number or email address below.  There also will be information on how to request records on the website listed below.  You will be asked to complete and sign a medical records authorization form in order to release your records.

After those 6 months (Oct 1-), all remaining medical records will be transferred to a professional medical records custodian that will maintain the records in accordance with applicable laws.  Updated instructions for requesting records will be available through the contact information listed above.
