Illumination Foundation Medical Group (Clarke Lew Medical Corporation) permanently closed on March 30, 2026 and no longer operates at its former clinic location.

If you are unsure who your current primary care provider is, or which medical group you may have assigned to, please refer to your insurance card or contact your health plan for further assistance.

Urgent Medical Needs

This practice is permanently closed and is no longer providing medical care. If you have a medical emergency, call 911 or go to the nearest emergency department. For urgent medical needs, please contact your current healthcare provider or your health insurance plan for assistance.

Medical Records

Medical records remain available upon request.

Please email clew@ifmedicalgroup.com with your name, date of birth, and a brief description of your request.

If you are unable to email, you may leave a voicemail at (949) 243-7788 with your name, date of birth, telephone number, and the reason for your request. Instructions for obtaining your medical records will be provided.

Future Medical Records Custodian

Medical records are in the process of being transferred to a long-term records custodian. Once this transfer is complete, this website will be updated with the custodian's contact information and instructions for requesting records.
